For CME activities that will include a self-assessment component, please use the template below to submit the post-test questions that adhere to the following requirements:
o A minimum of 4 questions per SA credit hour is required, 25% of which must be case-based in order to measure competence.
o Each question must have 4 possible answers. True-false responses, and responses that include “all of the above,” or “none of the above,” are not permitted.
o Post-tests must be completed by learners within 10 days after the conclusion of the CME activity.
o Learners must obtain a performance level of at least 75% on the SA post-test. If 75% is not achieved, the learner must have the option to re-take the post-test in order to receive SA credit.
o Note, Lunch sessions need four (4) questions – one being case-based. Pre/add-on sessions need sixteen (16) questions, four being case-based
